FAQ:
Is hospice care only for cancer patients? No. Cancer is only one disease that can limit life. An individual with any type of disease, acute or chronic may be eligible for hospice services. We serve individuals with lung, heart, liver, kidney, AIDS, Alzheimer’s, Dementia, stroke, and other diseases; many of which are considered chronic or have been debilitating over a long period of time.
What kind of services do you provide?
· Your Primary Care Physician will remain involved in your care as a vital part of the team through communication with your case manager or HWC Medical Director.
· HWC Medical Director oversees the care provided by the hospice team and offers suggestions that may be beneficial to the patient and/or staff for improved quality of life.
· Nurse Case Manager coordinates the individualized care plan based on the patient’s goals. He/she makes visits based upon the individual needs and a nurse is available to assist with an emergent need 24 hours a day, seven days a week by calling 301-791-6360.
· Social Worker provides emotional support, counseling, and coordination of community resources. He/she works closely with family/friends, assisting as needed to help ensure a healthy well-being during this challenging time.
· Certified Nursing Assistant assists with personal care and light housekeeping services under the direction of the nurse case manager.
· Therapist & Ancillary Staff –dieticians can counsel individual and family/friends about nutritional needs. Physical, occupational and speech therapists can work to accomplish specific goals to enhance an individual’s quality of life.
· Chaplain assists in providing spiritual care for the individual and their family/friends. HWC is not a denominational organization; however we do recognize the spiritual needs that some may have regarding the end of life’s journey.
· Bereavement Counselor supports the family/friends for 13 months after the death of a loved one. Individual and group grief counseling is available for hospice families and community members.
· Volunteer provides emotional support, short periods of respite care, companionship, massage therapy, hair styling and much more.
